Individuals, parents and legal guardians can get their official immunization records from the Citywide Immunization Registry (CIR) and check which vaccines they may need. To search for your or your child’s immunization records online, you can use your IDNYC number, NYS Department of Motor Vehicle (DMV) Driver/Non-Driver License number, the primary contact's mobile phone or email address.

IDNYC is a government-issued identification card that is available to all New York City residents age 10 years and older, regardless of immigration status. It is accepted as an official ID to access services and programs offered by City agencies. Apply for an IDNYC card.

If you are a parent or guardian, and your child's health care provider has listed you as the parent/guardian and primary contact in the CIR you can get your child's record. Any time you or your child receives a vaccine, be sure to ask the provider to update or confirm the parent/guardian contact information in the CIR.

NYC health care providers must report all COVID-19 vaccinations to the CIR; other adult vaccinations may be reported according to New York State (NYS) Public Health Law and the NYC Health Code. Pediatric providers are required to report all immunizations administered to children 0-18 years. Immunization records for people born in NYC before 1995 may not have complete information.

If you are fully vaccinated but lost your COVID-19 Vaccination Record Card or need verification of your vaccine status, you can request a copy of your COVID-19 vaccination record. You cannot get a replacement COVID-19 Vaccination Record Card at this time, but you can get an official record noting you have been vaccinated.

If you cannot obtain a vaccination record online using My Vaccine Record , you can apply for a record by completing the following form:

You can submit this form by completing the online NYC Bureau of Immunization Record Assistance Request Form and uploading a valid photo ID. Staff will respond as quickly as possible, usually within 2 business days.

Alternatively, you can mail or fax the completed application along with a valid photo ID to the fax number or address provided on the form. You may also call 311 to request that a copy of the application be mailed to you. Record requests by mail or fax take 1-2 weeks to process.
