Individuals, parents and legal guardians can get their official immunization records from the Citywide Immunization Registry (CIR) and check which vaccines they may need. To search for your or your child’s immunization records online, you can use your IDNYC number, NYS Department of Motor Vehicle (DMV) Driver/Non-Driver License number, the primary contact's mobile phone or email address.

IDNYC is a government-issued identification card that is available to all New York City residents age 10 years and older, regardless of immigration status. It is accepted as an official ID to access services and programs offered by City agencies. Apply for an IDNYC card.

If you are a parent or guardian, and your child's health care provider has listed you as the parent/guardian and primary contact in the CIR you can get your child's record. Any time you or your child receives a vaccine, be sure to ask the provider to update or confirm the parent/guardian contact information in the CIR.

NYC health care providers must report all COVID-19 vaccinations to the CIR; other adult vaccinations may be reported according to New York State (NYS) Public Health Law and the NYC Health Code. Pediatric providers are required to report all immunizations administered to children 0-18 years. Immunization records for people born in NYC before 1995 may not have complete information.

If you are fully vaccinated but lost your COVID-19 Vaccination Record Card or need verification of your vaccine status, you can request a copy of your COVID-19 vaccination record. You cannot get a replacement COVID-19 Vaccination Record Card at this time, but you can get an official record noting you have been vaccinated.

If you cannot obtain a vaccination record online using My Vaccine Record , you can apply for a record by completing the following form:

You can submit this form by completing the online NYC Bureau of Immunization Record Assistance Request Form and uploading a valid photo ID. Staff will respond as quickly as possible, usually within 2 business days.

Alternatively, you can mail or fax the completed application along with a valid photo ID to the fax number or address provided on the form. You may also call 311 to request that a copy of the application be mailed to you. Record requests by mail or fax take 1-2 weeks to process.

  • Certification Statement
    1. Read the Certification Statement and check the box to agree.
    2. Confirm your relationship to the person whose record you are requesting.
    3. Click "Next."
    4. If you are looking for your own record, see instructions below for Immunization Record Search - Your Own Record.
  • Parent/Guardian

    If you are a parent or guardian, and primary contact enter your information.

    1. Enter your name and date of birth.
    2. Enter your home address, email address and phone number(s).
    3. You can choose to authenticate your identity by either submitting your IDNYC number, NYS Department of Motor Vehicle (DMV) Driver/Non-Driver License number, or having a code sent to your mobile phone or email. Please update your provider with your current mobile phone and email address to enable this function.
      • If you select IDNYC:
        • Enter your IDNYC number. This will be used only to confirm your identity. The CIR will not save this information.
        • Check the box to allow the CIR to verify your IDNYC Card number.
        • Click "Next".
      • If you select New York State Driver’s License:
        • Enter your license number (driver or non-driver).
        • Click "Next".
      • If you select mobile phone number:
        • Enter your mobile phone number.
        • Enter your email address.
        • Click "Next".
        • You will receive a code sent as a text message to your mobile phone.
        • Enter this code and click “Next”.
      • If you select email:
        • Enter your email address.
        • Click "Next".
        • You will receive a code to your email.

  • Immunization Record Search: Your Child's Record

    Enter your child's information:

    1. Enter your child's name, date of birth and sex assigned at birth.
    2. Enter your or your child's Medicaid number or CIR number, if available. These fields are optional but will help make the search more accurate.
    3. Enter the first name, maiden name and date of birth of the child's mother.
    4. Enter the address and phone number reported to the health care provider at the time of the vaccinations.
    5. Click "Next".
  • Immunization Record Search: Your Own Record

    Immunization records for people born in NYC before 1995 may not have complete information.

    If you are searching for your own record and you are 18 years of age or older, enter your information:

    1. Enter your name, date of birth and sex assigned at birth.
    2. Enter your Medicaid number or CIR number, if available. These two fields are optional but will help make the search more accurate.
    3. Enter your mother's first name, maiden name and date of birth.
    4. Enter the address and phone number reported to your health care provider at the time of vaccination.
    5. If the search found more than one record:

  • Check your information and try again. To help us locate the exact record, enter as much information as possible.
  • Click "Next".
If the search did not find a matching record:
  • Check your information and try again. If no records are found, contact your or your child's health care provider or school. Call 311 for help finding a low- or no-cost immunization clinic if you need to receive vaccines.
